Checklist of Information to Bring to the Meeting
To ensure a productive consultation, please bring the following documents and information to the meeting:
Personal Identification: If you are acting on behalf of someone else and have been appointed as their Enduring Power of Attorney, kindly bring a certified copy (from your solicitor) and a copy of your photo ID (e.g., driver's license or passport).
Relevant Documents: Please bring any applicable documents to the meeting. The checklist below will guide you on the types of documents we are looking for.
CHECKLIST:
□ Copy of photo ID (e.g.: driver's license or passport)
Aged Care Details:
□ Copy of Copy of (ACAT) Aged Care Assessment Team and (ACAS) Aged Care Assessment Service
□ If in residential care, a copy of the contract/agreement
Details of Real Estate:
□ Market value – if time, ask a real estate agent for an appraisal or estimate
□ Amount of any mortgages outstanding
□ Costs to maintain (e.g., rates notices, insurance policies) or sell
□ Details on credit card debts, or other personal loans (including loans to related parties)
Income:
□ Proof of pensions/investments/superannuation
□ Personal Tax returns
□ Budget (estimate of living expenses)
Other Investments:
□ Balances for bank accounts/term deposits
□ Managed investment statements
□ Investment bond/debenture documents
□ Latest statements for income stream products (and a “Centrelink Schedule” if available)
□ Latest superannuation statements for each fund
□ Share certificates or latest statement for shareholdings
□ Details on any funeral bond or prepaid funeral expenses (including cemetery plots)
□ Latest statement for any life insurance policies
□ Market values of ‘other assets’ (cars, boats, caravans, etc.)
□ Details of any foreign income or super pensions
□ Details of any gifts made in the last 5 years. If you or your partner gift money, income, or assets, it may be assessed in your income and assets tests. How much you can gift and how gifting will affect you is helpful to our discussion.
Rental Properties:
□ Council rate notice for each property, showing property valuation details like Capital Improved Value (CIV) Site Value (SV) Value of the land
□ Details on any amount owing on each property
□ Details on rental income
Private Trust or Company:
□ Business/trust income tax return
□ Latest financial statements including profit and loss statement/s and balance sheet.
