Starting 1 July 2025, older Australians will have access to a revamped aged care system designed to help them stay independent in their own homes. The Support at Home Program, announced by the Australian Government, will streamline existing care options, providing more flexibility and tailored support. Here’s what you need to know:
What is the Support at Home Program?
The Support at Home Program is a government initiative that combines multiple existing in-home aged care services, including the Home Care Packages Program and the Short-Term Restorative Care Programme. It aims to simplify access, reduce wait times, and provide better care to older Australians who wish to live at home safely and independently.
Who is eligible?
The program is open to:

People aged 65+.

Aboriginal and Torres Strait Islander people aged 50+.

Individuals aged 50+ who are at risk of or experiencing homelessness.
Eligibility will be assessed through a new Single Assessment System using an Integrated Assessment Tool to identify support needs and develop personalized care plans.
What services are included?
The Support at Home Program offers a wide range of services grouped into three main categories:

Clinical Supports: Nursing, allied health, and other therapeutic services to maintain health and manage conditions.

Independence Supports: Personal care, assistive technologies, and home modifications.

Everyday Living Supports: Help with domestic tasks, gardening, meal preparation, and social engagement.
What are the new features?
The program introduces several innovative features:

Assistive Technology and Home Modifications (AT-HM) Scheme: Upfront funding of up to $15,000 for tools like walking aids and home safety improvements.

Short-Term Support Options: Including a Restorative Care Pathway (up to 12 weeks of intensive support) and an End-of-Life Pathway (support for individuals with less than three months to live).

Self-Management Options: Participants can choose their service providers, schedule their care, and even manage their budgets for greater control and flexibility.
How will funding work?
Funding is allocated based on individual assessments, with budgets provided quarterly. Unspent funds can roll over (up to certain limits) to accommodate changing needs. Contributions depend on the type of service and the participant’s financial situation. Clinical supports will be fully funded by the government, while everyday services like gardening may require higher participant contributions.
What happens to current care recipients?
Current recipients of Home Care Packages or the Short-Term Restorative Care Programme will transition automatically to the Support at Home Program. Existing funding levels will be preserved, ensuring no one is worse off due to the changes.
